Endoscopic Mini Caldwell-Luc 수술법

배경: 만성 부비동염에 있어서, 상악동내의 병변은 단순한 점막 부종에서 상악동 내의 비용 형성까지 다양한 형태를 나타내고 있으며, 특히 심한 부종과 동반된 비용이 공존 하는 경우에 있어 상악동의 경비적 중비도 개방술에 의한 술식으로는 상악동내의 병적 점막의 제거가 용이하지 않은 경우가 있다.

목적: 이에 기존의 Caldwell-Luc 수술법을 대체할 수 있는 상악동의 수술법을 고안하여 적용하였다.
재료 및 방법: 부비동 수술을 시행한 30명의 환자를 대상으로, 경비적 중비도 개방술에 의하여 상악동의 병적 점막의 제거가 용이하지 않은 경우를 적응증으로 하였다. 내시경용 상악동 천자 trocar와 cannula(외경 5mm)를 이용하여 치은협부점막을 통한 상악동 천공을 시행하고, 구강 점막의 천공부를 20mm로 수평절개 후, 견치와의 천공부를 punch forceps로 5x10mm의 크기로 열어주어, 외경 4mm의 내시경과 내시경 수술용 ethmoid straight & upward cup-forceps(외경 4mm이하)를 동시에 삽입하여 상악동내의 병변 조직을 선택적으로 제거하였다.

결과: 수술중 견치와를 통한 최소한의 점막절개와 골편제거로서 상악동내의 점막 병변, 특히 상악동내 하벽 및 전벽의 병변을 쉽게 직접 내시경 시야에서 특수한 기구를 사용하지 않고도 제거 가능하였으며, 수술소요 시간도 경중비도법으로 상악동내 점막 병변의 제거시 보다 크게 늘어 나지 않았다.

결론: Mini Caldwell-Luc 수술은 경중비도 상악동술식의 보조적인 방법으로 매우 유용하였다.

Maxillary sinus seen from middle meatal opening.
1. The antrum is explored by sinoscopy through canine fossa.

2. If the antral pathology cannot be removed easily though middle meatal antrostomy opening, the sinoscopy opening is then enlarged by punch forceps with a size of 5x10mm.
　
　
3. The antral pathology is removed with endoscope viewing through MMA opening or canine fossa opening.
