Backgrounds
simple ventilation and drainage of maxillary antra-seems not effective for severe pathology
transnasal removal of antral pathology sometimes not easy
Objectives
To enhance managing antral pathology
To avoid unnecessary adjacent normal
tissue removal or maceration during transnasal procedures
Materials and Methods
30 maxillary sinuses-chronic sinusitis with
severe antral pathology(male 17, female 13, age 15-67)
endoscopic endonasal sinus surgery with mini Caldwell-Luc
procedures

1. The antrum is explored by sinoscopy through canine
fossa. 2. If the antral pathology cannot be removed easily though middle meatal antrostomy opening, the sinoscopy opening is then enlarged by punch forceps with a size of 5x10mm. |

Conclusions
Endoscopic mini Caldwell-Luc operation was effective adjunctive procedure in ESS for managing maxillary sinus pathology which was not accessible from middle meatal antrostomy.
Endoscopic mini Caldwell-Luc operation did not show any significant complication, such as facial numbness or oroantral fistula, but the long-term follow up is warranted for the bony changes of the maxillary sinus walls.
